Clinical Observation on 20 Cases of the Internal Derangement of the Knee

Abstract
In the light of the traffic accident increasing, it seems justified to evaluate the knee injuries, so that it may add to improve both the diagnosis and treatment. It has often been proved that the diagnosis at the emergency room fails to disclose the exact location and extent of the injury.
Clinical observation was done for twenty patients with the knee injuries admitted and treated at the department of orthopedics from the period from January to July of 1971. Actual number of the joint structures involved in these twenty patients was thirty-seven.
The followings were the result,
1. Traffic accident was the cause of injury in thirteen (65%) of all cases, and industrial machinery was the next frequent cause. incidence was higher it male (70%) than in female (30%), and patients in their fourth and fifth decade were liable to have the traffic accident, while men in their second. decade the athletic injury.
2. The knee was positioned at the time of injury in the abduction, flexion and medial rotation of the femur, the valgus strain being, imposed upon them in, the half of all cases.
3. The Anterior cruciate and the medial collateral ligament were the structures: injured most of all (twenty out of 37 structures). The femoral attachment was vulnerable side in the anterior cruciate ligament and the injury of the medial collateral ligament showed even distribution of both bony attachments and its substance.
4. It has been reaffirmed that the early surgery is the treatment of choice in acute knee injuries and an exploratory arthrotomy in doubtful cases is strongly recommended for prompt and adequate repair of the lesion and restoration off the maximum functional recovery.
